Communities are increasingly challenged with rising health care costs, chronic disease management and addressing provider shortages. As a result, it is critical to find innovative ways to improve the health of communities. Community Health Workers (CHWs) are a cost effective bridge between patients with the greatest needs and the primary care physicians and other health care providers. Because CHWs know their community and are more likely to be trusted, they are able to address the health disparities and special needs. This webinar will explore the strengths of the CHWs and ways they can be added to a team to more effectively address the needs of the community.
