The ongoing success and longevity of a grant project is dependent on the sustainability of the rural health network that is implementing it. Sustainability is defined as the network’s ability to achieve desired outcomes and maintain the ability to do so over time. The ability to do so is a goal of FORHP grant funding.

The purpose of the Network Sustainability Action Plan Toolkit, comprised of a guide, template, and sample, is to provide a framework for rural health networks to actively plan for sustainability as network leaders deploy their networks’ business plans. Join the webinar to learn about the three phases to completing your Network Sustainability Action Plan.
